Use this form to enter your personal data to obtain a library card. The library collects this information to check if you are a resident of Anytown and that you are not mis-representing yourself. While the library's materials are available and freely shared, the library must assure that everyone can use the items. We will use the contact information you provide to send you reminders to return the materials by the due date, or to contact you to collect the materials when they are overdue.
